Dana Chapman, BSW, CMC
Certified Aging Life Care Manager
Dana credits a beloved social work professor and her civic minded parents for guiding her to her first internship in a small rural nursing home. She began working in special education and juvenile probation before spending the next fifteen years in Victim Advocacy. While serving a community rape crisis center she was honored with the Georgia State Governor’s Criminal Justice Coordinating Council Eagle Award for Outstanding Advocacy.

Her early career included hospital social work, elder care management, hospice social work, guardian ad litem work for Superior Courts and co-leading recovery groups for addiction and domestic violence victims.
​
In 2011 Dana became the Executive Director for The Guest House, Inc a nonprofit Day Care and Activities program for senior adults and those with dementia. After six years of service there she became the very first Executive Director for a Hall County nonprofit specialty transportation service for seniors who no longer drive.
​
Earning her Certificate in Care Management in October of 2020, Dana is thrilled to be working with Premier Care Management of Georgia in service to seniors and their families. Dana uses her extensive community connections, rich work experience and tender heart to help seniors and their families navigate, plan and maximize available services and support for older adults.
​
Dana is deeply involved in her community, through nonprofit boards and volunteering. She was honored by Woman Source, a career and personal development network, with their 2014 Community Hero Award. Currently she serves on the Executive Board for the North Georgia Senior Resource Network and is an active Gainesville Kiwanis Club member.
